Painful periods: primary and secondary causes

Many people experience some cramping with periods. But pain that is severe, worsening, or interfering with your life is worth looking into, it should not simply be endured.

This page explains the difference between primary and secondary period pain and when to seek advice.

Key points

  • Some period pain is common; severe or worsening pain is not something to just endure.
  • "Primary" pain has no underlying condition; "secondary" pain does.
  • Endometriosis and adenomyosis are important secondary causes.
  • Pain that disrupts your life is worth reviewing.

Primary vs secondary pain

Primary period pain occurs without an underlying condition and is related to the normal menstrual process. Secondary pain is caused by a condition such as endometriosis, adenomyosis, or fibroids.

Symptoms worth noting

Pain that is getting worse, occurs outside your period, or comes with other symptoms is worth reviewing.

  • Pain that stops you doing normal activities
  • Pain during sex or when opening bowels
  • Pain outside of your period
  • Heavy bleeding alongside pain

Diagnosis: what to expect

A specialist may take a history, examine you, and arrange imaging such as ultrasound. In some cases, further assessment for endometriosis may be discussed.

Options

Options depend on the cause and can include pain management, hormonal approaches, or treatment of an underlying condition. These are decided together with a clinician.

When to see a specialist

If period pain is severe, worsening, or affecting your life, it's worth seeking review, particularly to check for conditions like endometriosis.
